Buttocks Augmentation
Buttock enhancement aims to beautifying the body as seen from behind.
Buttock augmentation can reshape, increase the volume, and lift the gluteus. The results are that of more harmonious buttocks, with the desired shape and volume, and a better and perkier appearance.
The Buttocks are the centre of the posterior body and play a very important role on its contour. The buttocks on the back play the same role of the breast on the chest. The main features of both are the projection and the round shape, something strictly confined to women.
Buttock reshaping and augmentation procedures alter and improve the appearance of the male and female Buttocks. This is a relatively new field in aesthetic surgery, but in later years this type of operation has become popular especially in big cities like Rio deJaneiro and Los Angeles. Many patients feel uncomfortable with their posterior contour and troubled by situations like swimming, beach or sports. Also, changing dressing fashion, codification of aesthetic norms of the buttock, as well as procedural improvements have increased interest in buttock augmentation surgery.

The Best Candidates:
An area of the body not easily enlarged, shaped by weight training or diet is the buttocks region.
Individuals that wish to enlarge, lift, and reshape the buttock mounds, have 2 options:
- Enlargement with fat transfer (your own fat)
- Or using a variety of different size and shape implants from which to choose.
The decision is made depending on each person’s individual desires and on their general body build. Generally, anyone in average physical condition or good health can be a candidate for buttock augmentation surgery.
The surgery
Fat transfer: It is a minimally invasive procedure that involves fat suctioning from other part of your body, purifying it and transplanting it into the buttock area to look as natural as possible for the optimal cosmetic effect. It is usually done under general anaesthetic as a day case, will go home the same day, quick recovery time and less painful than buttock implants.
Buttock Implants: It is a bigger operation but gives you permanent results. It involves a 5-7cm single incision is placed over the tailbone in the midline, through this incision the buttock muscle (gluteus maximus) is identified and dissected to create just large enough pocket for the implant. The implant will be inserted and positioned to look natural.
The patient will require to stay in Hospital overnight. Pain medications typically are prescribed to help with the discomfort. Some patients will require drains overnights.
There is usually minor swelling and bruising after buttock augmentation and the results will be noticeable immediately after surgery. The procedure is done under General Anaesthesia or/and epidural anaesthetics.
After Surgery
Fat transfer: Quick recovery, pain usually tolerated with simple analgesia.
Buttock implants: The patient may be very uncomfortable for the first few days. It is difficult to turn in any position to get comfortable. It is advisable that someone will be with the person for the next several days just to help get out of bed, prepare meals, etc. It will take few days before the patient find it more comfortable to walk about and get back to daily activities.
The dressing should remain dry and intact for few days. The patient will be reviewed at the clinic 7 days after surgery for wound and implant check.
Getting you back to work and daily activities
Fat transfer: Back to work in 2 days time, daily activities in 7-10 days.
Buttock implants: Full activities including gym, work, bicycling, and running can usually be resumed about four weeks following surgery. It may take up to six or eight months until the results feel as if they're part of the body and the patient forgets about them even being there.
Buttocks Reduction and Recontouring
Aesthetic surgery of the buttocks encompasses many forms. Patients may seek to enhance buttock shape with liposuction, buttock lift, buttock implants or fat transfer procedures. Patients with traumatic buttock injuries and contour deformities from injections also require correctional procedure, often with fat injections, and autologous tissue flaps. The dramatic increase in body contouring after massive loss associated with bariatric surgical treatments for obesity has also extended to buttock contouring. Weight loss producing flat buttock deformities are often severe in nature and pushing the need for buttock enhancement surgery.
It is also important to realise the buttock reshaping surgery should address the whole back, flanks, hip and thigh as one unit, it is a rather back, buttock and thigh remodelling procedure.
Recent studies in evolutionary biology have suggested a strong correlation between the "hour-glass" figure and general physical and psychological health. This correlation is best summarized by an ideal waist-to-hip ratio of 0.7 that transcends cultures, is temporally stable, and is cross-generational.
The function of the buttock musculature in stability and gait is an important consideration and often has an impact on the procedure chosen.
No procedure should be carried out on the buttock area or the posterior contour without a full assessment and complete understanding of the complexity of it. Patients seeking treatment should be made aware about the potential risk factors associated with any surgical procedure.

The consultation
It is fundamental to understand the differences between the female and male buttocks. A womens buttock is characterized by curvy lines, round volumes and prominent projection, while a mens buttock characterized by straight lines, square volumes and side depressions. Sagginess of the buttock should not be present neither full gluteal crease which may indicate early ptosis. Full assessment and knowledge of all the above is vitally important and should be brought to the attention of the patient prior to surgery. The fatty fullness under the crease, what we call banana deformity, Cellulite-contour irregularity of the skin and the saddle bag has to be addressed. Buttock contouring surgery should address all the above points to be able to produce desirable results that match our patients expectations.
The surgery
There are different techniques suiting different patients for buttock contouring surgery. They include minimally invasive procedures, liposuction, buttock lifting procedure, Buttock enhancement with lipofilling or implants procedures.

Buttocks Liposuction
Liposuction is the most commonly performed plastic surgery in Australia. This technique is used to reshape the body by permanently removing localized fat deposits. Generally, the suctioning is performed in the low back above the buttocks, the back of the thigh just below the buttock fold or on the hip at the side of the buttock. The goal is to create a natural rounded curve to the buttock that recalls the look of the slim, well-proportioned athletic body.
The tendency to accumulate fatty deposits on the buttocks may be inherited, and these deposits are often resistant to diet and exercise. Many patients become discouraged at having lost unwanted pounds without seeing a significant change in this area. The overall proportions of the body may be unbalanced, and the extra bulk can keep clothes from fitting well. In these cases, liposuction can successfully trim excess fat to bring better balance and symmetry to the body.
Liposuction is a great way to get rid of those bulges that you cannot seem to get rid of any other way. If’ you’re considering buttocks liposuction, the following information will provide you with a good introduction to the procedure.
The Consultation
During the consultation, you’ll be able to talk to your Plastic Surgeon about the changes you would like to make in your appearance. Dr Barnouti will explain the different options available to you, the procedure itself, its risks and limitations.
Your surgeon will begin with a physical examination to evaluate your overall weight, your skin and muscle tone, and the fat deposits in various parts of your body. He will also record your complete medical history, including any medications that you are currently taking.
Keep in mind that the higher your goals, the more you will have to do to achieve those goals. If you are just concerned about removing fat, liposuction alone will achieve that goal. But if you want an athletic, sculpted look, and you want to look great in a bikini, you may need to incorporate extensive exercise into your plan.
Be sure to ask all the questions you have about the procedure, and ask to see photos of the surgeon’s recent liposuction surgeries, before and after treatment. Learning everything you can about your options, risks and benefits is the key to making an informed decision.
The Surgery
The surgeon will mark the precise areas of the body where the fat is to be removed. An intravenous (IV) line will be inserted in a vein in your arm to make sure the fluid level in your body remains in balance. You or your surgeon may opt for general anesthesia, or you may be local anesthesia and a sedative for relaxation.
During the procedure, the surgeon makes a tiny incision in the skin, typically in or near the buttock crease, and inserts a thin tube called a cannula into the fatty area. The cannula is used to break up the fat deposits and sculpt the area to the desired proportions. The unwanted fat is suctioned out with a vacuum canula.
In the super-wet technique, a saline solution containing a local anesthetic and adrenaline is injected into the area to be treated, which makes the fat deposits easier to break up and extract. This extra fluid also minimizes trauma to the surrounding tissue, reducing swelling and post-operative pain. The administration of adrenaline also decreases bleeding during surgery, further reducing risks.
The tumescent technique, in which even larger amounts of liquid solution are injected, has similar benefits.
A relatively new method, water jet soft liposuction, uses water jets to liquefy the fat and numb the area followed by fat suction. Like the super-wet and tumescent techniques, water jet technique minimizes trauma, bruising and blood loss. It is particularly useful for small and moderate amount of fat.
Your surgeon will choose the best technique for you. His choice will be determined by a combination of factors, including the precise area to be treated, the amount of fat to be removed and your preferences.
After The procedure
For a day or two, you can expect to feel tired, as though your body has been through a workout – and it has! Your hips and legs will be stiff and store, and you may experience some pain, burning, swelling, bleeding or temporary numbness. Your will be prescribed simple analgesic medication to relieve this initial discomfort.
Sometimes a small drainage tube is inserted under the skin to drain excess fluid. Your doctor may prescribe an antibiotic ointment to prevent infection at the incision sites. Your stitches will be removed in a week to 10 days, unless they dissolve on their own.
You will be fitted with a tight compression garment to be worn for up to six weeks after your surgery. This specially designed garment will help reduce pain and swelling, and help your body settle into its new shape.
Getting you back to work and normal activities
Most of the bruising and swelling should subside within three weeks. You will be able to begin light activity as soon as possible. Return to work within a few days if your work is fairly sedentary. You should avoid strenuous activities for 4 weeks as your body heals, but within 4-6 weeks you should be able to resume all normal activities.
It’s important to continue wearing the compression garment, which should be easily hidden under your clothing, for4-6weeks.
